When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office. Please be sure also to tell us of any medications you are taking as well as the name of your general physician.

 

Your initial exam will include comprehensive records that utilize the latest in dental technology, evaluation of your gums, teeth, chewing muscles, temporomandibular joint (TMJ), and an oral cancer screening. We’ll answer all your questions and address your concerns and design a treatment plan specifically tailored to you. After your time with the doctor, our front office team will go over the proposed treatment plan and provide estimates.

If you have questions or would like to discuss Invisalign treatment, obstructive sleep apnea and snoring treatment, cosmetics, or would just like a second opinion on a treatment plan that was proposed by another office, come see us! We offer no charge consultations and second opinions.
